Where your records are kept
Booking, scheduling, intake forms, insurance billing and records are handled through Headway's systems. This website doesn't collect or store health information.
How health information is generally used
- Treatment: to evaluate, diagnose and treat you, including sending prescriptions to your pharmacy and coordinating with other clinicians involved in your care.
- Payment: to bill your health plan or you for services.
- Health care operations: to run the practice, for example scheduling and quality review.
- As required or permitted by law: for example, to prevent a serious threat to health or safety, or when a court order requires it.
Most other uses, such as marketing, need your written permission, which you can withdraw later. Psychotherapy notes, if any are kept, receive extra protection. Florida law may give additional protections for mental health information.
Your rights
- See and get a copy of your health records.
- Ask to correct your records.
- Ask to be contacted in a certain way or at a certain place.
- Ask to limit what is used or shared.
- Get a list of certain disclosures of your information.
- Get a paper copy of the full notice.
- Choose someone to act for you, such as a personal representative.
- File a complaint if you think your rights were violated.
Complaints
You can complain to the practice by calling (786) 442-0548, or to the U.S. Department of Health and Human Services Office for Civil Rights at hhs.gov/ocr/complaints. You won't be retaliated against for filing a complaint.
